Hi, there have some problems in the EA form.

1. No Perkeso = No IC but no "-"
exp: ic=970101-10-1010
Perkeso = 970101101010
you can put that automotive.

2. in section D, no 4(a) (Jumlah tuntutan potongan oleh pekerja melalui Borang TP1 berkaitan:Pelepasan)
should key in the SOSCO

3. in section E
KWSP= maximum is RM 4000

4. in section F
name and Alamat Majikan
should put the name and address, no just put the address.

3. Er... I know the max is RM 4000 for now, but what's the point of filling in the max?
